      <title id="t-130d8268d372">Introducción</title>
      <p id="p-84977a2a404b">El papel del control intensivo de la glucosa en personas con diabetes tipo 2 y enfermedad cardiovascular (ECV) preexistente es controvertido.</p>
    </sec>
    <sec>
      <title id="t-7a68cb8953cc">
        <bold id="s-7159882937e4">Objetivo</bold>
      </title>
      <p id="p-7a01ec5eb944">Determinar en un subconjunto de personas con diabetes tipo 2 y ECV preexistente, el efecto cardiovascular del control intensivo de la glucosa frente al tratamiento estándar.</p>
    </sec>
    <sec>
      <title id="t-2b9f36cf0a4c">
        <bold id="s-458ef914b27d">Métodos</bold>
      </title>
      <p id="p-bd90ca21d0de">Se realizaron búsquedas en Medline, la biblioteca Cochrane, EMBASE y la base de datos de registro de ensayos de los Institutos Nacionales de la Salud en busca de ensayos controlados aleatorizados que evaluaron el efecto del control intensivo de la glucemia versus el control estándar de la glucemia en personas con diabetes tipo 2 en la ECV incidente. Los datos fueron extraídos utilizando una formulario estandarizado. Cuando los datos no estaban disponibles en las publicaciones, se estableció contacto con los autores.</p>
    </sec>
    <sec>
      <title id="t-0c92c45bbc22">Resultados</title>
      <p id="p-c07f90268892">Se incluyeron ocho ensayos con 8.339 participantes. Entre los adultos con diabetes tipo 2 y ECV preexistente, no hubo diferencia en el riesgo de desarrollar nuevos eventos cardiovasculares en aquellos asignados al control intensivo de glucemias en comparación con aquellos en el grupo de atención estándar (riesgo relativo 0,98, intervalo de confianza del 95% 0,87 a 1,09).</p>
    </sec>
    <sec>
      <title id="t-1e01a7b88074">Conclusiones</title>
      <p id="p-115d3ba4a454">En las personas con diabetes y ECV preexistente, el control intensivo de la glucemia versus la atención estándar tuvo un efecto neutral en la incidencia de nuevos eventos cardiovasculares.</p>
    </sec>
    <sec>
      <title id="t-3d956d9aa50e">Comentario</title>
      <p id="p-7445f679f759">El control glucémico en pacientes diabéticos ha desmostrado reducir las complicaciones microvasculares: retinopatía, nefropatía y neuropatía periférica<xref rid="R44719910023578" ref-type="bibr">2</xref>,<xref rid="R44719910023579" ref-type="bibr">3</xref>,<xref rid="R44719910023580" ref-type="bibr">4</xref>,<xref rid="R44719910023581" ref-type="bibr">5</xref>. Sin embargo, hasta el día de la fecha no se ha comprobado de manera fehaciente que el control glucémico reduzca la incidencia de eventos cardiovasculares<xref id="x-6c902a03767d" rid="R44719910023625" ref-type="bibr">6</xref>. En esta revisión sistemática de alta calidad se investigó en el subgrupo de pacientes de mayor riesgo: aquellos que ya habían tenido ECV. En el análisis principal, como en los análisis de sensibilidad y de subgrupos, no se  evidenció una reducción de la incidencia de eventos cardiovasculares. Si bien existen otras medidas eficaces para prevención de este tipo de  eventos en personas con diabetes (por ejemplo, mediante la reducción de la tensión arterial) es importante resaltar este hallazgo negativo para definir junto a los pacientes las metas de control glucémico en base a la valoración individualizada de los desenlaces clave, entendiendo que el principal efecto del control glucémico no incluiría la prevención de eventos macrovasculares.</p>
